Symptoms: Coheed holds a lack of pleasure in everyday life. He is very easily irritated and thrown into fits of rage. Thoughts of self-harm and harm to others. No motivation to do anything other than lay around. A wide range of mood swings that are nearly encompass their own personalities. He is terrified of large anthros.

Biography: His Mother was a diagnosed schizophrenic. His father was never home. He was deeply attached to his Mother who passed away when he was eleven. The majority of the symptoms began taking place around that time. His grandfather was Bi-polar to an extreme degree. He knows nothing about his father by any means.

Medical History: He is deathly allergic to tomatoes and olives.

Has the patient been referred by a court ruling?: No
If so, Why?: His application to be admitted to the mental hospital was done by his grandparent's who were now unable to take care of him anymore from his mood swings and self-harm.

Has the patient been previously diagnosed with any mental or health illnesses?: Yes
If so, what?: He is diagnosed with Schizophrenia and Social Anxiety.

Has the patient been known to harm them self or others (or both)?: He is known to do both, though harm to others is a little less consistent.

Privileged Patient?: No/ Guard Escort

Dr. Araceli Morganti has observed the patient has mood swings and is not medicated, however, is at least aware of the issues he has and is looking to cope with them. Has been given a notebook to write journal entries in.

Patient had a mood swing upon talking about Sana. Major depression caused him to try to claw at his eyes.
Patient was given a stress ball in attempt to prevent further self injuries.
Patient did weird hand/arm movements in the air, claiming that it were only out of boredom. (Added October 17, 2011)

Patient was brought in for a check up after an extended period of time passed since his last session. Patient appeared incredibly stressed and attributed this to extreme loneliness and tension. Patient began to claw at his eyes, explaining that he felt compelled to do so every time he became depressed. Patient was asked if there was anything that made him happy in order to give him some other kind of release, and he mentioned the outdoors.

Patient has been given permission to venture to privileged areas of the hospital, as he has had no recorded incidents.

Patient was also introduced to the day room, as he did not know where to socialize previously.

Patient was in day room hassling a visitor. After throwing a book at a guard, patient was asked to leave the room and ignored the request. Upon being lightly touched, the patient got incredibly violent and was subdued and placed in room.

Patient was brought in, initially depressed about the previous incident in the day room. He admitted that in this state he believed he was a completely different person. Though he had no memory loss, patient describe that he felt like he was 'watching' himself outside of his own body. Patient was then questioned whether he kept up with the journal Dr. Morganti had given him and stated he did, and although it was nearly full, did not recall ever writing in it.

Patient gave me his journal, which will be analyzed for final proof of Dissociative Identity Disorder.

Patient was called in order to discuss the results of his journal examination. Patient appears to have a fetish for blood as well as attachment issues, though the largest issue at hand was that the patient does have Dissociative Identity Disorder. Patient is aware his alters exists, but doesn't appear to have any strong communication links with them, which will be tackled next session.

Dr. Allen Carter's report:

Patient brought fellow patient Leo Anderson to the infirmary following an injury to the latter in the garden. After some questioning he admitted to licking the wounds clean of blood. I instructed him not to do this as it could be dangerous for someone's health but he claimed that he 'had to do it' and 'couldn't help himself'. This is possibly something that should be looked into in a session with his therapist.

Session called in order to discuss his most recent incident. Patient described that he saw the wounds and couldn't help himself from licking him, and although he says he was helping, he also admits that his logic at the time was flawed. He also admitted to having an odd fascination with blood and attributed this to his father's abuse, that with so much exposure to blood, he eventually came to like it. Patient attempted to be reassured, but fainted shortly thereafter. Best hypothesis is that he fainted due to remembering his past, which should be brought up in a later session.

Patient was then sent to the infirmary and seemed to be mentally stable upon waking up. Patient wanted to speak with patient Leo Anderson and left to do so.

Patient was brought in for a group session with fellow patient Leo Anderson as a follow up to a short talk I had with them after observing them spending a lot of time together in patient Anderson's room. I asked questions to ascertain the nature of their relationship (if everything was consensual and such) and informed them of a few rules which, seeming to be in a relationship, they should be aware of. Patient Deftinwolf seemed very agitated through most of the discussion, refusing to speak for parts of it and demanding a quick end to it when I asked further questions.

Patient came into my office accompanied by fellow patient Leo Anderson requesting permission to spend the night in Anderson's room. After consulting my colleagues and reviewing the patients' records (Deftinwolf's in particular) I came to the decision that it would be too dangerous for Anderson and denied the request. When the reasoning was explained to them patient Deftinwolf became very upset and was led out by patient Anderson.

Patient was brought into the infirmary after an altercation with patient Leo Anderson in his room. Reports from the guards and orderlies on the scene state that patient Deftinwolf seemed to be in a different alter from his usual one at the time of the incident. It seems that Anderson was panicking and Deftinwolf forcibly pulled him out from hiding and made threats. Patient Anderson hit him and knocked him back: resulting in a fractured nose and a concussion. Patient was kept overnight in the infirmary for observation.

I request that the patients remain separated until they have spoken with their therapists regarding the issue.

Patient was brought in after his incident and seemed incredibly testy. Once patient was informed of a guard escort for his actions, he switched to another alter and became very aggressive and threatening. When switching back, the patient seemed reluctant but still accepted the fact he needed a guard. Judging by the patients attitude, there are dependency issues that should be addressed.

Patient is required to have a guard escort at all times.

Patient was assaulted by patient Leo Anderson in the garden. Anderson bit Deftinwolf on the shoulder, breaking the skin. Deftinwolf was brought to the infirmary for stitches.

Guard's report:

Patient was present when fellow patient Leo Anderson went into a fit of some kind. This panicked Deftinwolf and sent him into a rage. He seemed to be trying to protect Anderson and became violent with the guards. Patient was quickly sedated and subdued, taken to the infirmary for minor injuries he'd gotten while in his panicked state.

Patient was brought in after his altercation in the day room. He was allowed to recover from the sedation and checked for injuries. Once he'd been checked and determined to be in good health he was moved to isolation to await speaking with his therapist.

After deliberation with Patient Leo Anderson's therapist Doctor Stephen Caprioli, it has been decide that Patient Leo Anderson and Patient Coheed Deftinwolf are to be separated except for monitored meetings at set times during the day and evening, which will take place in the largest therapy room. Touching will be limited in order to prevent aggression at the end of these sessions.

Both patients will be worked with until they are at an appropriate level to have free communication again.

Patient was in day room conversing with Orderly Carna Yin Valzon. Patient believed the women to be his mother as well as believed he was elven years old (possibly another alter). When told the orderly was not his mother, patient went into a panicked state and eventually passed out.

Patient called for a session in order to start integration therapy. During the course, he mentioned he had a violence fetish, but was told this was nothing too out of the ordinary. Patient then questioned how to begin integration therapy, and was given a journal in order to begin communication between himself and his alters. Patient asked if there was anything else before he would freely be able to meet with Patient Leo Anderson, to which he was informed that dependency was an issue, but would be addressed in later sessions.

Patient called a session in order to documented his progress. Patient had since filled his journal and was specifically talking to his bully alter, who dubbed himself "Jesse." Patient stated he has been in much more control and finds himself much more motivated to get better, mostly in relevance to Patient Leo Anderson.

Patient was able to switch to "Jesse," and the alter agreed to integrate into Coheed due to believing he had no more purpose to serve. It is unknown if the patient's third alter still exists at this point in time, though the childlike alter was no dangerous.

Patient has made great strides and should be allowed to see Patient Leo Anderson freely soon, assuming Patient Leo Anderson has also made enough progress. Following sessions to touch on patient's blood fetish and dependency.

Patient has done tremendously well in holding his progress. When speaking of his violence affinity, he admitted that it was a purely sexual desire, and mentions that he only feels such a way when with his partner, Patient Leo Anderson, and mentions a lack of control. In order to seal this final issue, I believe a group session should be done with Patient Leo Anderson, if the patient is ready for such.

Until then, Coheed has been advised to work on his social skills. While not crippling, patient has anxiety towards social situations, and has agreed to work on opening up to others.

Patient was called in for routine physical examination. Patient's vitals are healthy but patient shows signs of difficulty seeing from his left eye. Recommendation has been filed for a more thorough eye exam to check for astigmatism in the left eye.

Patient has shown significant progress and is ready to prepare for leaving the hospital. Patient showed nervousness at meeting new people, though the anxiety is not crippling and can be attended to with outpatient care, if the patient so chooses.

Patient will meet with Lassa Hayes in order to prepare himself for life outside. Patient's eye will most likely be addressed outside of the asylum.

Patient has been doing quite well with his therapy and lessons and is scheduled to leave soon. Due to his unfortunate circumstances, patient will be set up with a small apartment and be further assisted outside the asylum to get him adjusted easier.
