Weight: ~103 lbs.

Symptoms: Sometimes sees things, but that isn't too often at all. To her, she tends to 'blank out' and sort of 'wake up' in alternative locations, caused by a switch in personality. She remembers nothing during the time one of the alternative personalities have surfaced.

Biography: There's a bit of a history of MPD in her family, what with her father having it as well as his mother. He also suffered from Schizophrenia. It reached on further in the line, but that's unimportant. Her mother developed an addiction to a multitude of dangerous drugs once she and her husband divorced. Thu was only two then, but she kept in contact with her father. When she would spend the weekends with him, she would often be subjected to his terrible fits. Normally he was a very kind man, but during said fits he could become very abusive and cruel. Thu never told her mother all those horrible events, instead keeping them to herself and returning home as per usual, using typical excuses to justify the bruises and such. Slowly, she began to talk less and less before she just stopped talking altogether.

When she turned five, it seemed she began to show signs of MPD as her father had. This came about by her having bit her baby brother quite a bit, who was about 2 and a half at that time. Plugs were torn out of him, tiny plugs from her tiny teeth, but chunks all the same. This, at first, seemed just a very violent display of anger toward her brother, but she sat there cackling like a madwoman the entire time. It stopped abruptly and she began to freak out. She had explained to her mother, who at first wasn't so sure if she was lying or not, that she remembered nothing.

She realized her daughter wasn't lying when it happened again. And again. And when she attempted suicide again and again. When she began climbing things she shouldn't climb, doing ridiculous and dangerous things she deemed as fun. And also when she became very angry and verbally violent. She provoked a lot, attempted to get a rise out of others and start fights.

At 7, Thu's mother didn't want to deal with it anymore. She wanted to succumb to the drugs and not have to worry about her strange little daughter. She decided to give her ex-husband full custody of their son. Afterward, she began the processes of admitting Thu to a children's psychiatric hospital. She was admitted to Franciscan Hospital for Children until she turned 14. From there, she went to McLean Hospital until she was 19. The two kept in contact for a bit, though since she was raised in these sorts of environments, these were the closest things she had to home. Before she turned 17, her mother had left for Florida, moving to Melbourne. With her mother's help (who had yet to seek rehab), she found East Haven Mental Hospital and sought to admit herself.

Medical History: Anemic.

Has the patient been referred by a court ruling?: No.

Has the patient been previously diagnosed with any mental or health illnesses?: Multiple Personality Disorder

Has the patient been known to harm them self or others (or both)?: Yes. One of her alternative personalities tends to be very self-destructive, while the other is incredibly violent toward others. This one tends to bite. A lot.

Notes:
Personalities have been observed as follows:

1. A very hyperactive, giggly, almost childlike personality. In this state, she tends to do very reckless things, break things out of sheer klutziness, and so on. For example, she will run on top of tables and jump from one to another. Very mobile, very hyper, very happy.
2. A very depressed, incredibly self destructive and suicidal personality. In this state, she will attempt to either hurt herself or kill herself with any available resources nearby.
3. An incredibly angry, provoking personality. She doesn't initiate fights, but she will say things to try to invoke them.
4. A very psychotic, sadistic, even cannibalistic personality. She will say strange, surreal things and attempt to thrust herself onto someone in order to try biting plugs out of them. If she does succeed she will, as disgusting as t his may seem, eat it.

Current Medications: Iron Supplements

The patient was involved in an incident with Ryan Summit in which she bit Ryan. Staff member 'Mason Conrad' pulled her away to prevent any severe damage. The patient was tranquilized and sent back to her cell.

Patient assault patient Synchron Jue in the cafeteria, and was quickly subdued and returned to her room until calmed down. Patient is now required to be supervised by a guard when entering public areas of the asylum.

Patient was reluctant to 'speak' at first, though eventually complied and gave details of her past before forming personalities. She claims that she was told she couldn't feel excited, angry, or sad, and forced herself to feel nothing at all. However, it is mostly likely these qualities materialized into her separate personalities. Next session will be set to begin therapy.

Patient was found in garden and had assaulted Patient Lorelei Fabre. Patient was compliant when guards on scene arrived.

Patient will once again require guard escort when exploring the hospital.

Guard's report:

Patient was subdued with the help of patient Lorelei Fabre after she had assaulted patient Ross Sharpe (biting his neck). She was knocked out in the process, receiving some injuries and was brought to the infirmary.

Dr. Allen Carter's report:

Patient was brought into the infirmary after an altercation. Upon giving a physical examination it appears that the patient received a neck fracture (no spinal cord damage it seems), and a high grade concussion. Brain swelling is likely and so I had the patient moved to a nearby hospital for more specialized tests and treatments.

Patient was called in due to staff reports of the patient having amnesia. When brought into a session, it was discovered to be true; patient's demeanor was different than anything previously exhibited by her alters. Patient could also not recall any previous events and did not acknowledge any mental issues stemming from them. Patient was given a summary of her past and will be monitored over the next few weeks.

Patient was called in for a group therapy with 'Ross Sharpe' and 'Lorelei Fabre'. We discussed the tension exhibited between the patient and Mr. Sharpe. Thu explained that she was afraid to lose her friendship with Ross, and sad about what she did. The two patients talked it out and the tensions was eventually soothed.

Patient was brought in and initially wanted to speak of her current artistic block. After a while, suspicions about a minor short term memory loss were confirmed. Currently the memory loss does not affect her daily performance so there is nothing to practice on, so attempts to improve memory will be done through improving health.

An iron pill will be given to the patient daily, as well as number of meals and content of meals will be monitored in order to assist with weight gain.

Guard's report:

Patient attacked patient Ross Sharpe in the game room. She bit his neck and drew blood. She attempted to escape and the guard present chased her into the library. He received some minor injuries from her struggling but was eventually subdued when she entered the hall. Patient has been brought to isolation until her therapist can speak with her and her patient rights have been suspended until further notice.

Guard's report:

Patient was in the cell hallway speaking with patient Karn Colton. They were speaking low so the specifics of what they were saying could not be heard. Patient Colton left to go to his room when Hazlitt lept onto his back and bit his shoulder repeatedly. He tried to shake her off by hitting her into the door frame. Hazlitt had to be subdued by the guards stationed in the hallway and was taken to isolation until she could speak with her therapist.

Met with patient in isolation to discuss a guard escort to prevent future incidents. Integration therapy will also begin soon.

Patient was also speaking, though whether this relates to a boost in confidence or a simply lack of pencil and paper was not discussed.

Guard's report:

Patient was attacked in the cafeteria by patient Karnald Colton. She was punched in the face repeatedly until guards intervened. She was brought to the infirmary where it was determined that the injuries were only superficial bruises.

Patient was brought in in order to discuss long term treatment. Patient was speaking, noting that because she was capable of speaking, she should use it instead of writing. Patient was told of integration therapy and seemed more curious than offset and agreed to attempt to begin communication through writing to her other alters.

Dr. Allen Carter's report:

Patient was involved in an accident in the garden. She apparently fell off one of the benches and landed on her head. She was brought into the infirmary unconscious. She was sent to a nearby hospital to better examine any possible brain damage she may have received as there were few external injuries (a scrape, bruise). It was determined that she had some bleeding on the brain and when she awoke it appears that she is suffering from amnesia and may have some serious visual impairment now. But further testing will be needed to determine the exact nature of her vision.

Patient was called in for a session and did not appear to have any signs of depression despite her recent accident. Patient was mostly inquired about her alters to which she inquired that although she did hear them on occasion, she attempted to ignore them. Patient stated she would attempt to work on speaking with them on her own time.

Patient was discovered in the garden having allegedly attacked patients Dakota Reynolds and Lenna Ellis. Patient was sedated and returned to her room. Patients Dakota Reynolds and Lenna Ellis did not request medical attention.

Guard's Report:

The patient was biting patient David Jones at the day room. The two were separated before Jones was sent to the infirmary. Although Jones seemed entirely submissive, it is recommended that this incident be addressed in a future session.
