Symptoms: Mitchel Patrickson, hereby referred to as 'patient', was admitted a nearby medical facility underweight with multiple self induced lacerations to forearms and inebriated. The patient was admitted after his legal guardians found him unconscious because of blood loss. Upon regaining consciousness the patient was severely agitated and attacked an attending nurse. The patient was restrained and put under sedation. After his release, his parents have requested an involuntary admission to our facility, East Haven Mental Hospital, for psychiatric assessment and treatment until such time when he is deemed safe for release.

Biography:
The patient lives with his parents, who are his full time guardians. They have reported that starting a year prior his grades had dropped severely and the patient has repeatedly been suspended for fighting and has faced truancy repeatedly. The parents report that the patient has become steadily more aggressive since this started and has withdrawn from contact both with his peers and family. The parents claim to have no prior knowledge to previous suicidal tendencies nor an explanation for the numerous scars indicating that self injury is not a new development. They have also mentioned that he was caught stealing several months ago but there has been no further incidents.

Medical History: The patient has no known medical issues or allergies.

Has your character been referred by a court ruling?: No - The patient has been put in an involuntary psychiatric hold by request of legal guardians.
If so, Why?: N/A

Has your character been previously diagnosed with any mental or health illnesses?: The patient has no previously diagnosed medical or mental illnesses.
If so, what? N/A

Has your character been known to harm them self or others (or both)?: Yes. The patient shows signs of being a danger to himself and those around him. The patient has a history of self injury and was admitted after a failed suicide attempt. The patient also has a history of violence and fighting while attending school, resulting in disciplinary action (i.e. suspension).

Anything else?
[Mitchel has brown hair, grey eyes and borders the line between slightly effeminate and masculine. His wrists, feet, legs, stomach and back are covered in numerous scars from his cutting. His drugs of choice are alcohol and nicotine.]

[The truth: Mitchel has been a victim of severe bullying, thus his issues with truancy and fighting. Furthermore, his parents are severely neglectful and often leave him by himself for large spaces of time. It's rare that he has any supervision. The self injury has been going on since he was fourteen and developed as a way to control something in his life that quickly developed into an addiction. To keep his addiction quiet and also to feed it he began to steal bandages and blades from nearby stores. Mitchel also has difficulty concentrating, shows signs of irritability and agitation as well as aggressive behavior. He also shrinks from physical contact, shows poor social skills and shows signs of depression. His low weight is indicated to an undiagnosed eating disorder that began similarly to how the self injury started.
Mitchel also has auditory and visual hallucinations. So far these hallucinations have been minor and he's been able to keep these a secret. However, as his mental state continues deteriorates these hallucinations will become stronger and contribute to his growing paranoia and delusions.]

The correct diagnosis once all symptoms finally present themselves is Paranoid Schizotypal along with anorexia. However, upon admission this will not be apparent. Possible misdiagnosis is severe depression caused by stress/bullying.]
