Symptoms:
- Extremely poor nutritional health
- very brittle hair and nails from constant washing and bathing
-Body covered in self inflicted rashes cuts
-Hands and feet are dry and raw from over washing
-Panics during any sort of physical contact
-light sensitivity can not handle much more light than what’s given off from a desk lamp

Biography:
Jonesy had a very normal childhood. Grew up in a middle class home, got good grades, and played sports with the local kids. How ever he developed a bad case of OCD around the age of twelve. Jonesy would go out of his way to keep from touching people and he washed his hands at least eight times a day at the least. He was also quite dead set on trying to wash the brown tint out of his blond hair because with his parent’s dyed platinum blond hair, he always thought that his own hair color was dirt. These problems were never treated.

This escalated to the point where he had to move out of the house when he was seventeen because no one he lived with could handle how clean and sterile he wanted his house to be. Around this time his OCD developed into automysophobia.

At the age of eighteen Jonesy left his house in order to pick up cold medicine because a recent drug but in his town that was converting over-the-counter cold medicine into drugs so he could no longer have it delivered right to his house. He was abducted right there in the parking lot and sexually abused. From that point forward Jonesy developed full on agoraphobia and would not even step foot out of his house for the next eight years.

Now his past time can be seen as spending up to nine hours a day trying to scrub dirt and grim off his body that is not actually there.

None of these mental problems were discovered until Jonesy started falling into debt. When his inheritance ran out at the age of twenty five his bills started stacking up untill his house went into foreclosure. When the authorities came to remove Jonesy from his home he was found sitting in his bath tub scrubbing his arms untill they bled.

Medical History:
He has a nutritional deficiency that has lead to his highly compromised immune system.
Not much else is known from his health before had because he never left the house. Even to see a doctor.

Has the patient been referred by a court ruling?: Yes. Jonesy was sent to a East Haven after a court ruling saw him unfit to live on his own as he is considered a danger to himself.

Has the patient been previously diagnosed with any mental or health illnesses?:
At the age of 12 he was diagnosed with OCD. He has never been back for a mental evaluation since then but has since developed and extreme case of both automysophobia (the fear of being unclean) and agoraphobia (the fear of leaving the home and going into social situations).

Has the patient been known to harm them self or others (or both)?:
He was sent to the mental hospital because of his self destructive tendencies but he is no danger to others as he can’t bare to touch or be touched by others.

Current Medications: Anti-biotics

Dr. Allen Carter's report:

Patient was treated for rashes (seeming to result from overwashing) on various parts of the body, mainly the hands and arms. A topical cream was applied and patient should be instructed to wash less often or use lotion (I've already requested this to be provided to the patient). Patient is also to be given antibiotics for the next three weeks to ease the transition of their weakened immune system to this less sterile environment. Once his immune system is deemed strong enough he will be given booster immunizations.

Patient was first brought in showing obvious signs of anxiety and wished to go home. Patient asked for a bottle of hand sanitizer on the desk and doused himself in it. Patient was anxious the entire intake and named off many reasons for his fear, though they were quite outlandish.

Patient requested to be put out for the search and medical tests, and also requested that his room be bleached, or that he be allowed to choose the cleaners, though both these requests were denied for health reasons.

Patient was brought in for a session and has currently not adjusted yet. Patient has admitted to requesting his room be cleaned more, but staff have refused due to hospital policy.

Patient was spoken to about his body, and it was clear he knew nothing about his immune system, and the patient was awfully fascinated about all the details and appeared to get comfortable for short moments until he began to discuss dust mites, in which the patient became anxious once more.

Patient has permission to have dust supplies under the condition they can only be used every 45 minutes, and the vacuum cleaner cannot be used past 9 pm. If either of these conditions are broken, they are to be taken away until next session.

The patient called a session concerning his confiscated vacuum. The conversation quickly evolved to talk about his cleaning issues. The patient seems to think that he 'will die' if he isn't clean. When informed about how his over-washing can prove to be hazardous to his health, he seemed to be under the impression that 'blood is the body's soap'. I further explained how he was misinterpreting the actual purpose of blood, although he seemed reluctant to accept the idea. I have recommended him to shower less often and with a shorter duration. For now, he'll be restricted to showering only twice a day for a maximum of an hour long each shower. If he breaches these conditions, the shower will be locked until the next (and immediate) session. Furthermore, I have decided that he'll be allowed a small quantity of lotion soap in his room. He will be allowed to use the vacuum, but under the same conditions.

In addition, I recommend for all doctors to remove any cleaning supplies from the room before beginning a session.

Patient initially came in very angry over the new limit he received, but quickly calmed down as the session progressed. Patient was informed of how the constantly and long bathing was in fact, detrimental to his health, and lightly educated on how skin works in disease prevention. Patient, although a afraid of germs, seemed willing to touch the book I brought as well as allow me to touch his hands. Patient seems to now understand why it is so important, but admitted that the limit was too sudden, and assured that with unlimited time, he would be able to limit the time he needs to spend in the bath to 1:30 within a week.

Patient has been given unlimited time once more to pursue this, though he was told that if he does not keep his word, the original limit will be placed again in order to keep his skin healthy.

Patient came in claiming he was bored, and seemed a little lonely despite not admitting to such. Patient requested a female guard escort in order for him to feel comfortable in the day room for him to watch TV and integrate himself into the patient community. Patient was given a list of when the part time female guards are available.

Patient's fear of males is attributed to an event where he was kidnapped in and tortured in front of an audience. To be touched on at a later date.

Patient requested a session to announce that his self-monitored attempts at reducing his cleaning habits have been successful, specifically stating that he has been able to limit himself to one bath per day. He also announced that he was able to make some friends, specifically naming Chief of Security Sana Shepard and Patient Ross Sharpe.

However, he did mention that he still could not touch people, and felt incredibly dirty for doing so. We began some exercises where the patient would shake my hand and have to wait 15 seconds before he was allowed to wash it. Patient was able to do this successfully once, but was still incredibly nervous. To be continued in a later session.
