Patient Record: Amanda Destiny Corless
Name: Amanda Destiny Corless
Gender: Female
Species: Human
Age: 22
DOB: January 25, 1989
Height: 5'10''
Weight: 150 lbs
Symptoms: Antisocial behavior, hallucinations (visual and auditory), prone to drug addiction, social anxiety, generalized anxiety disorder, delusional thought, compulsive liar, belief in magic and aliens, obsessive rumination, fits of psychotic episodes in which she's also plagued with illusion or derealization.
Biography: Had loving parents, though her father was addicted to opiates. her paternal grandmother lived with the family and tormented the girl, as well as caused strife between her and her sister. She is resentful towards her grandmother and believes she is still haunted by her spirit.
She was considered antisocial in school despite having a small group of friends. She was considered to be a genius based on standardized testing scores though she graduated high school with a C average. She later became addicted to benzodiazepines (psychoactive drugs commonly used as anti anxiety medications).
At twenty she went into a rehab only to come out pregnant. It's unclear who the father was, because her only explanation was 'them'. She was abducted by 'them'. She tried explaining to doctors that the baby inside of her wasn't just a baby, it was something much bigger than they could ever imagine. She revered her baby as one would a god. A week after the baby was born and dubbed Elijah Enoch Corless he died from SIDs. This fueled her delusions further and perhaps sent her off the edge.
Medical History: Galbladder disease makes her diet limited to very little red meat dairy and grease, she's given birth two weeks before being sent here so her body and stitches are still healing
Has the patient been referred by a court ruling?: Yes
If so, Why?: When doctors where discussing causes for her infants death she experienced a psychotic episode. She accused the doctors of killing her infant and became violent. She was arrested and later deemed unfit to stand trial.

Anxiety disorder
Has the patient been known to harm them self or others (or both)?:
Is easily antagonized into fighting but does not have a history of intentionally causing harm to others, or herself.

Re: Patient Record: Amanda Destiny Corless
Written by Mason Conrad:
The patient tells that her newborn was killed by her doctors. She explains that the shots given to the baby weren't immunization as she initially thought, but rather shots to actually kill the baby. She holds the belief that the doctor's motive to kill the baby was due to the fact that her baby was 'special', and that 'they knew'. When questioned about why she thought the shot was actually what killed her child and how the doctors knew her baby was 'special', she went back to when the whole ordeal began. During her stay within a rehabilitation center, the patient claimed to be abducted. While describing the abduction, she told that she saw the scenery in a 'very bright' fashion, and that 'the people from the sky' were the ones who were responsible for this 'abduction'. As she described the situation, she exhibited a fluctuating amount of anxiety and eventually broke down into tears. Because of this, I resorted to comforting her and sending her out to complete the rest of her intake.
The events as described above gives evidence to her hallucinations and anxiety listed within her symptoms.
Follow up session required.

Re: Patient Record: Amanda Destiny Corless
Dr. Kayoko Saito:
Patient was first brought in and reported that she saw the spirit of her grandmother in a hallucination that perhaps taunted her, but she quickly dropped the subject. She also reported that generally in the afternoons, she has been have onslaughts of complex thoughts that physically pain her that she attempts to sleep off. Further in the session, it is most likely that these episodes began at the death of her father, which she claims was a suicide. She also reported that while she was pregnant she was able to better control these episodes because she was concentrating on becoming a mother.
In order to create a different form of concentration, the patient was asked for some things she did to help get her mind off things. One of the things mentioned was musical composition, and she also informed that she has equipment available for her to do this. Will be calling patient's mother soon in order to have it delivered so she will hopefully have some sort of release.
Contemplating on assigning anxiety medications, but difficult considering the patient's past drug use. To be decided in a later session.
