Biography:
Azrael was born and raised with Sekura Senthos (as friends, not brother and sister), who happens to be another patient. Due to his natural physical capabilities and finesse with his hands, the man grew up to become a professional boxer. During his first few years as a boxer, Azrael helped raise Sekura's baby. It wasn't until the child was killed in a truck accident did Azrael begin to show signs of multiple personalities. One by one, each of his personalities were created as his own way to 'escape' the death of Acel (the name of the Sekura's child).

Dr. Kayoko Saito:

Patient had not had a session in months and went to check on him. He is still displaying a very sociopathic persona, though his concerns were primarily with his personalities, whom he hadn't seen in months. Patient claims they come out during mental vulnerabilities and has not had any situations where a switch would be necessary. Follow up session to be held by myself or Dr. Araceli Morganti.

Dr. Kayoko Saito:

Met with patient concerning his form of antisocial personality disorder. He can name the specific incident where he believes it formed, but cannot comment on the nature of it. Many scenarios run by him, though he admits he had never taken them into account and has requested to think about it for the next session.

Dr. Kayoko Saito:

Patient was called into a session today and told that he has a message written in his notebook that said "Back to Business," and recognized the handwriting as one of his alters that he refers to as the 'god' (used to describe his behavior, not worship). The patient has no reported hearing voices since transferring but is concerned his personalities are becoming active again as a result of his friends, Sekura Senthos and Synchron Jue, being discharged. Patient was unable to name any specifics about his personalities. Either Sekura Senthors or Synchron Jue will be contacted for details so these can be discussed with the patient next session in order to find a working solution for his disorder.

Dr. Kayoko Saito:

Spoke to Synchron Jue and Sana Shepard concerning the patients personalities. Together they describe a total of 5 as follows:
- One that tends to show symptoms of a sociopath.
- One that is very confident and shows sadistic tendencies. Patient refers to this as the 'God.'
- One that is uncontrollably depressed.
- One that knowledgeable yet egotistical. Patient claims this is a persona that reflects how he acted his freshmen year in high school and refers to this as the 'Nerd.'
- One that acts incredibly laid back and smooth.

Two other personalities are possible as reported by Sana Shepard, being one that stumbles around drunkenly and another that is filled with rage, though the patient couldn't confirm either.

Patient was at first uncertain of what to make of it, then later asked what the next course of action was. Patient was informed that we will begin therapy in order to integrate his personalities together. Near the end of his session he appeared to switch to his 'God' personality and shortly left the room thereafter.

Dr. Kayoko Saito:

Patient called a session in order to help manage his alters and was introduced to the idea of integration. Patient seemed uncomfortable with the idea, and was given a full explanation as well as an option to speak with a person that has been through it. Patient agreed. More updates later.

Dr. Kayoko Saito:

Patient met up with outpatient Synchron Jue in order to discuss a cure for his disorder. Patient seems reluctant to accept the idea that all his alters are indeed his, and his 'god' alter seems equally reluctant to integrate. More sessions planned in order to alleviate the anxiety and hopefully begin the process.

Dr. Kayoko Saito:

Patient was called in to discuss his integration more and agreed to the treatment, requested that the 'nerd' be integrated first. After a bit of questioning, patient informed that he was able to speak to his alters via journal. Journal was brought in and the 'nerd' was contacted via journal. Eventually patient agreed to switch to the 'nerd,' who stated that he was alright with integration and would help with the process by teaching patient Azrael Boyka how to communicate.

The 'nerd' also confirmed that the personalities displaying rage and drunkenness were indeed separate alters.

Patient attacked a guard in the training room with weights and managed to successfully knock him out. Upon trying to leave, patient assaulted another guard on scene with a stolen taser and managed to subdue him as well, though was quickly taken down once more guards arrived on scene.

Patient is in isolation until notice from doctor.

Dr. Kayoko Saito:

Visited patient in isolation, who initially had no recollection of any of the assaults. Patient then switched to the 'god' personality, who was the alter that committed the assaults. The alter explained that he was attempting to get 'Azrael' (assumed the host personality) to gain confidence and rise up against him so that he would be a stronger person. The alter then retrogressed back into a dormant state when explained the flaws in his plan. Patient was then allowed back into his room.

Privileged area access revoked until further notice.

Dr. Kayoko Saito:

Patient called session in order to discuss the nature of his personalities, in that none of them seemed particularly "good". Patient was introduced to the idea of "good" qualities being split up and was reluctant to accept the idea, however, got more warmed up to the idea of integration throughout the session. In the course, patient switched alters to the 'god' who claimed that he would not integrate as he wanted to see Azrael gain confidence with his own eyes. The alter was spoken to on the terms that he was only a 'god' in title, and he eventually retreated back into a dormant state.

Dr. Kayoko Saito:

Patient came in in order to speak about his social habits and how he didn't seem to connect with anyone. Patient seemed to lack confidence and have a very pessimistic view on the other patients, and was given some ways he could begin to connect with them.

Dr. Kayoko Saito:

Patient originally called a session himself in order to discuss the guilt he felt for his murders. Patient was spoken to and seemed to get better, and then informed that he was able to switch to his 'depressive' personality at will. The personality did not seem to know much of what was going on and throughout the session would switch to the 'rage' personality when anger was required. It appears these two alters are very closely linked.

Sana Shepard:

Patient attacked me in cell hall, but was quickly detained when guards arrived on scene. Reason for the assault is unknown.

Patient has been taken to isolation until further notice from doctor.

Dr. Kayoko Saito:

Met patient in isolation, who was in an incredibly emotionally compromise state, screaming and yelling as he tried to escape the table. The patient was evidently in the 'god' personality at this time. Patient did not speak much, although appear to feel incredibly defeated when spoken to about his recent failed result. Patient in the end switched out of this 'god' alter and back into the host personality, and was shortly thereafter released to his room and suggested to speak with his 'god' alter.

Chief of Security "Sana Shepard" was also sent by his room as the patient wished to apologize.

Dr. Kayoko Saito:

Patient was called in for a checkup and is doing quite well, and states that he has not been hearing any voices from his alters. Patient mentioned he was introverted, but seemed much more content with the fact. To be monitored for a few more days and if this streak, we will begin setting up a plan for discharge.

Dr. Kayoko Saito:

Patient has shown now signs of relapse and is thus being discharged later this week.
