Symptoms: Delusional, insecure, hallucinations, koutaliaphobia
Biography: Twelve years ago, Jack Cucchiaio became plagued by a demon known as the Ginosaji. He had to give up his job as a Forensic Pathologist because of his obsession with chasing and destroying this illusion. More than once he resorted to violence, going on merciless gun rampages through his hometown and around the world to escape from and kill his demon. When arrested for criminal behavior, he was sent to get help from Dry Times Mental Hospital. Is now in the process (hopefully) of a transfer to East Haven.

Medical History: No medical irregularities.

Has your character been referred by a court ruling?: Yes, but has since requested a transfer from his old Hospital.
If so, Why?: Multiple homicides.

Has your character been previously diagnosed with any mental or health illnesses?: None.

Has your character been known to harm them self or others (or both)?:
Both. Harmed self to try and get away from the Ginosaji, but the demon stopped him so he could be tortured for the rest of his life. Went on many gun rampages and tried many times to murder him, and ended up harming many civilians.

Privileged Patient?: No

Current Medications: Anti-Hallucinogens

Patient was involved in a conflict at the cell department. Information will be reviewed before speaking with the patient concerning the incident.

He was found attempting to choke patient 'Mestono Manson'. His displayed aggression was likely caused by his phobia of spoons.

Patient suffered from a hallucination during his attack against Mesteno Manson. The hallucination involved seeing Mesteno Manson as the Ginosaji. He will be placed under supervision when moving into any public area of the hospital. Furthermore, he will be dosed with the appropriate amount of anti-hallucinogens to help prevent any further incidents.
