Age: 17
DOB: May 3rd 1995
Height: 5'6
Weight: 105 lbs

Symptoms:
Olivia has been diagnosed with ocd and depression. She also sometimes has suicidal tendencies. Olivia sometimes can zone out as well. She is very calm even with all of this but she is timid at times. She is very talkative but she does not ramble on as she is shy.

Biography:
Olivia came from a very abusive childhood and had to be put n foster home after foster home. Her parents would beat her and not feed her much. The only thing her old parents cared about was beer and television and for her to leave them "the hell alone". When she was put into foster homes, she hated it. She felt no one cared about her or what she went through. She would scream, write on the walls, and get so depressed that she started cutting. She had been to the hospital many times. She has scars on her wrists. She had to go to the hospital so much that it was ordered that she be put in a mental facility. They deemed her far beyond help, even though she can be helped.

Medical History:
No health problems other than the multiple cutting incidents which made Olivia lose a lot of blood in the past.Other than that, no health problems.

Has the patient been referred by a court ruling?: Yes they have been referred.
If so, Why?: She has been referred because she has no parents and is in need of help per the systems decision to do so.

Has the patient been previously diagnosed with any mental or health illnesses?: Yes
If so, what? Ocd and depression.

Has the patient been known to harm them self or others (or both)?:
She only has been known to harm herself. She never wants to hurt anyone but herself.

Patient entered in in a calm state of mind and was able to answer all question. Patient believes she was brought against her will, though agrees that she does need treatment for her depression and suicidal tenancies.
