Name: Samantha White
Gender: Female
Species: Hyooman
Age: 25
DOB: 11/21/1986
Height: 5'4"
Weight: 138 lbs.

Symptoms: Emotional phases. Usually either very happy and motivated, or downright depressed, suicidal thoughts.

Biography: Samantha was born to a somewhat wealthy family, and grew up in the suburbs around the same town in which East Haven Mental Hospital is located. She was diagnosed with dyslexia in school and was ridiculed and bullied often because of her learning disability, by both her peers and her family.

This often put her into a state of depression (although her grandmother had bipolar disorder), and she'd begun to have mood swings over the course of a few months sometime during her teen years. However, this went away. She then went on to pursue piano as a career and moved away for four short years for college.

Being alone was quite freeing for her, but she was forced to return on account of not being able to support herself financially. Again, her family failed to give her any praise, and soon, her odd mood swings returned, only with a bit more severity.

This led her to attempt suicide once, but her friend had somehow rescued her by a convenient surprise visit. Although her life was indeed spared, her condition worsened, and her "happy phases," as she called them, were crazier than ever. Samantha knew that her depressive states would worsen, too, then. Afraid she'd try to take her own life again, she decided to seek help at the local mental hospital (as an outpatient), while trying to work on getting concerts and performances around town.

Medical History: Dyslexia.

Has the patient been referred by a court ruling?: No.

Has the patient been previously diagnosed with any mental or health illnesses?: No, as she never spoke of it all too often.

Has the patient been known to harm them self or others (or both)?: Not particularly, except for when she attempted to end her life.
