Patient Record: Mitchel Patrickson

Symptoms: Mitchel Patrickson, hereby referred to as 'patient', was admitted a nearby medical facility underweight with multiple self induced lacerations to forearms and inebriated. The patient was admitted after his legal guardians found him unconscious because of blood loss. Upon regaining consciousness the patient was severely agitated and attacked an attending nurse. The patient was restrained and put under sedation. After his release, his parents have requested an involuntary admission to our facility, East Haven Mental Hospital, for psychiatric assessment and treatment until such time when he is deemed safe for release.

Biography:
The patient lives with his parents, who are his full time guardians. They have reported that starting a year prior his grades had dropped severely and the patient has repeatedly been suspended for fighting and has faced truancy repeatedly. The parents report that the patient has become steadily more aggressive since this started and has withdrawn from contact both with his peers and family. The parents claim to have no prior knowledge to previous suicidal tendencies nor an explanation for the numerous scars indicating that self injury is not a new development. They have also mentioned that he was caught stealing several months ago but there has been no further incidents.

Medical History: The patient has no known medical issues or allergies.

Has your character been referred by a court ruling?: No - The patient has been put in an involuntary psychiatric hold by request of legal guardians.

Has your character been previously diagnosed with any mental or health illnesses?: The patient has no previously diagnosed medical or mental illnesses.

Has your character been known to harm them self or others (or both)?: Yes. The patient shows signs of being a danger to himself and those around him. The patient has a history of self injury and was admitted after a failed suicide attempt. The patient also has a history of violence and fighting while attending school, resulting in disciplinary action (i.e. suspension).

Patient came in seemed incredibly agitated and distracted, consistently scratching his arm when made to speak. Patient was very reserved that will perhaps improve with time to get situated in the hospital environment.

Patient reacted negatively to the strip search, but this is mostly normal.

Patient assaulted patient Lorelei Fabre in day room and was subdued. Patient Lorelei Fabre testifies that the patient believed he was being stared at and assaulted him for this reason.

Patient was called in for a session to discuss his violent outburst in day room. Patient gave the reason that Patient Lorelei Fabre was staring and it was 'rude' and 'creepy,' though would not explain exactly what bothered him so much. Patient was then question about his violent history and got incredibly tense when a specific event was hinted at, but calmed down when no information could be given on it, and then persisted on saying that nothing was wrong. Patient was then given a drink and the session ended.

Some mental illnesses hinted at, though as of now it seems premature to draw conclusions.

Patient will also have an orderly come by his room regularly in order to keep his nails trimmed.

Patient was brought in for a session and asked for coffee once more, though panicked at the site of the orderly Rozetta Stone that brought the drink in. Patient attempted to throw the cup as the orderly and then ran to a corner of room. After a bit of silence, patient attacked me, but was quickly subdued with a gaurd and taser.

To be touched on in a following session.

A small background check was made on the patient to his school. A few years ago he had excellent grades, but when he began to get bullied, his grades started to plummet and he got into many fights with his peers.

Patient at first seemed very calm and willing to speak, however, it was discovered the patient had been cutting himself with broken pieces of the mirror from his room. Patient insisted he was fine and eventually complied with allowing the doctor to see his scars. Patient assured the doctor that his other scars were from gang fights. When his bullies were brought up, he changed his story, claiming they were from fights with them. Patient insisted that he thought nothing of it and denied cutting himself previously, and insisted that this cutting was only a one time thing. Patient was then allowed back to his room.

Do not replace the mirror in this patient's room. This is for his safety.

Patient was apparently napping in the day room. A while later, patient has a sort of episode and pushed the table over in the day room before being subdued and taken to isolation.

Patient was brought in and was incredibly anxious, and was rubbing his arms, appearing to be attempting to break skin. Patient referred to himself as 'we,' but when questioned for the identity of the other parties, he denied he ever said 'we' and was not questioned on this for the duration of the session.

Patient began telling his story, though through pronouns, and thus most was left unidentified. Patient stated that 'they' had changed and would never leave him alone, and hinted that 'they' existed in some form in real life, perhaps at school or in his household, though there was no solid evidence. He also hinted at that he began to harm himself because 'they' told a certain truth he couldn't deny, despite how he disliked their message. Patient described his friends as tangible people at school, though the full identity of 'they' couldn't fall under this location. 'They' may be a series of hallucinations the patient is having, given all the locations they could be according to the patient as well as the duration of the ridicule. Patient began to get anxious when questioned on whether he really believed 'they' existed or not, and was not further pressed on the subject.

Patient also described 'he' in a very similar way, though formally identified this man at his father. Perhaps in addition to his school life being difficult, the patient received similar treatment in his household as well, which eventually lead to his suicidal tendencies and isolation from others.

Patient is extremely anxious and may be prescribed anti-anxiety medication in order to assist with this. Patient is likely also having audible hallucinations, as he never gave 'they' an identity or appearance, though this needs to be confirmed.
